Evaluation of the in-home telemonitoring for veterans trial

Trial participants were enrolled in the Coordinated Veterans’ Care (CVC) Program and monitored their chronic conditions from home using blue tooth devices. 

Data was collected from these devices and monitored in collaboration with their general practice.  This report outlines the findings after the conclusion of the trial.
