Qualifying Service Details Partner/Widow(er) of Non-Pensioner Veteran or Mariner (Australian, Commonwealth or Allied)

This form is used by the widow of a veteran who wants to claim Partner Service Pension but the veteran has not had their Qualifying Service previously determined. This is a supplementary form to be submitted with a claim for pension.
